This is an survey application that I built (of course with contributions of my colleagues), to remove the burden of writing separate sets of code for each and every survey conducted. So this might be useful for you too.

wasnt able to use the demo with Firefox 1.0.4 OSX Panther 10.3.8
choosing the “other” checkbox, wouldnt allow me to type in my answer in the input…
clint: Even i’m using Firefox 1.0.4, but it doesn’t give me that problem. I dont have access to a OSX Panther, so I won’t be able to check it out. Can you please check whether it generates a JavaScript error (from Tools > JavaScript Console) after you have clickd ‘Other’ checkbox/radio button, because the text field is enabled using JavaScript.
Hi Venushka, no js errors… I can click my mouse in the input but am not able to type anything and if I proceeed (next), then the form validation correctly kicks in with “Please specify your business (Question 1.2)”
I tried it with Safari 1.2.4 and it works great, just wont go in Firefox for me….
